Support Coordination
HICSS can assist you to implement your NDIS plan.
We provide assistance in:
- helping you understand the purpose of the funded supports
- access those supports as well as mainstream supports
- coordinate them to allow you to get the best outcome.
This frees you up and avoids the stress of having to navigate providers, supports and sometime the confusing world of NDIS. We provide you personalised budgets allowing you to maximise your plan budget. We’ll also give assistance and guidance on using your participant portal.
We deliver face to face services building a caring and supportive working relationship and can answer any questions you might have as we know it can be tricky at times.

What is Support Coordination?
- Helping you to link in with supports and assist in helping you make a decision
- Assist you in developing specific needs and goals
- Regular check ins to make sure your happy with supports and to make changes if need be to obtain the best outcome
- Assist in teaching you how to manage your own plan in the future
- Manage crisis situations as the arise.
- Assist with linking you in with mainstream supports
- Assistance with your participant portal
- Prepare you for your NDIS plan review, and follow up assessments you may require
- We offer a house 4 bedroom house in the East Maitland area. Close to local shops and public transport

We can assist you to:
- Source and provide you with a list of options with your local providers that best meet your needs
- Work with you to connect with the providers to individualise supports and develop supports to meet your needs and overseer your Service Agreements and supports so you know you’re receiving the best
- Assist you to develop the skills to learn to manage your supports long term
- Manage a crisis situation as they arise
- Regular check-ins and you will have your coordinators direct contact details to make it easier to connect with and able you to have a person-centred approach.
Levels of Coordination
Support connection – We assist you to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals.
Support coordination – coordination of supports: We will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage your service delivery tasks, live more independently and be included in your community.
